Non-operative management is considered the first line of treatment for knee osteoarthritis.

Exercises will focus on restoring knee function as it relates to improving your strength and tolerance to functional activities like going up and down stairs, standing up from a chair, walking, etc.

Shown are two simple strength progressions to help achieve these goals.

1. Squat Progression. Start by gently tapping your butt to a chair or box and stand back up. If you find this to be challenging or painful, you can reduce the range of motion and/or use your hands for assistance. If you want to make them harder, you can hold a weight in your hands. As long as it’s safe to do so, you can also remove the chair so you’re able to sit into a deeper squat.

2. Step Up Progression. Start as low as you need to while using your hands for assistance. Over time, you can do them without your hands and gradually increase the height of the step. If you have good balance, you can also stand sideways and practice lateral step downs while trying to keep your pelvis level. To make these harder, increase the height of the step.
