kacchan is never the type to say 'thank you'. most of the time, he'd rather show his appreciation through actions. the most you'd get is probably a 'thanks' in form of a grunt.
so izuku never thought he'd hear a soft 'thank you, izuku', coming from him.
kacchan has been sick since the night before and has only gotten worse. he could barely speak because of his sore throat and his temperature was so high they banned him from going to classes. izuku, as persistent as he is, decided to take care of him despite his disapproval.

don't get it wrong, izuku never liked it when kacchan is sick. he couldn't survive way too long without the explosiveness he adored ( fell in love with but he's not ready for that conversation ) but he'd be lying to not admit that kacchan IS cute when he's all docile and needy.
